In a brief synopsis of Sanam Re, Akash (Pulkit Samrat) is an ambitious young man who's always rebuked by his boss and asking for work even on 31st December on Akash (Pulkit Samrat). Akash has to visit his hometown to sell old photo studio as no one is there to take care of studio.

The photo studio is of his grandfather (Rishi Kapoor), Akash was so much influenced by his Grandfather. Akash is confused with his love life finds in his birthplace with his childhood love Shruti (Yami Gautam).

He tries to find Shruti at his hometown, but she left his hometown already. Suddenly one day Akash's boss call him and ask him to stay at his hometown not to come return back to office, as the project on which he is going to work, has been cancelled by Mrs Pablo. The reason behind project cancellation is Mr.Pablo ditched Mrs.Pablo and Mrs.Pablo file case in court, won the case in court and now she is own that company.

Now the Love triangle created while Akash went to Canada to get business contract from very sexy, hot, gorgeous and rich- Mrs Pablo (Urvashi Rautela) in Babaji's Ashram. At Canada Akash also find his first love Shruti to whom he dumped for his career growth in his earlier life. Shruti refuse his proposal of marriage at Canada and told him if their love is true, they will meet again. Akash couldn't get his business contract from Mrs. Pablo by telling her he doesn't do business with his friends. Other side Mrs Pablo is in deep love with Akash and she comes at India to meet to Akash, but Akash can't be able to forget Shruti. Mrs Pablo and Akasha start searching Shruti at different places where she have lived. Finally they could be able to find Shruti, but she denied to get marry and refuse his proposal at this time also. There is a reason behind why Shruti denied his proposal, she has last stage disease of heart. Love shows its power now, Akash donate his heart to Shruti to save her life and leave a letter, written as we will meet again someday.
